Posted:

ALPINE - Federal investigators now know what likely caused a fiery and fatal plane crash in Alpine last year.

The plane crash happened fourth of July weekend last year. According to the National Transportation Safety Board, it was caused by pilot errors and possible fatigue.

The NTSB report could not conclude for certain that the pilot was fatigued, but does say the flight was during the pilot's normal sleep schedule.

Five people, including three from Alpine and two from Midland, were killed in the accident. The air ambulance was taking a patient from the Alpine Casparis Municipal Airport and was on it's way to a Midland hospital.

The plane crashed shortly after takeoff.
